Waiver Form

If you are booked into one of our Events, you'll need to complete this waiver form prior to skating.  Once you have purchased tickets, you'll be emailed a link to our online form or you can print this one out and bring it with you.   This is an annual waiver and will cover you in our events for one year from the date of completing it the first time.
